Abstract

Introduction: The relationship between Iron deficiency and infection susceptibly has been investigated as well. We aimed to study the effect of iron deficiency anemia IDA on immunity in 401 children with IDA and 226 normal children.

Methods: The percentage of Neutrophils, Lymphocytes, the level of immunoglobulin’s and serum iron Ferritin, Hb, MCV, Ht, MCHC, and MCH were compared. Our study was conducted in the hospital in Sidi Bel Abbes region (west of Algeria) and the sample consisted of 627 children.

Results: Our result show that there was no significant difference in age between patients and control group (p=0.20). There was a statistically significant difference between all hematologic and immunologic parameters between the two groups (p<0.001). The percentage of Neutrophils was 71.09±5.02 in children with IDA and 58.74±8.40 in the control group (p<0.001). The percentage of Lymphocytes was 41.38±3.52 in children with IDA and 33.06±4.98 in the control group (p<0.001). IgG level was 1493.14±53.35 in children with iron deficiency and 1080.21±216.48 in healthy children (p<0.001). Significantly difference levels of IgM (194.57±33.38 in children with IDA to 141.23±38.52 in the control group) (p<0.001) and IgA (461.71±19.09 in children with iron deficiency to 278.87±90.63 in healthy children) (p<0.001) was observed between the two groups. A susceptibility to infections has been observed in children with iron deficiency in our study. Our investigations have shed light on Pseudomonas addition to its ability to synthesize and release enterobactin-related siderophores as a way of obtaining iron from their environement. We continued our experiments in order to characterize the concentration of siderophores produced by 30 specimens of pseudomonas isolated from 30 children’s with IDA and suffered from many diseases. In the same time, we found that the percentage of the siderophores was significantly suppressed by the addition of a Human serum in the medium of culture; it’s extracted from thus children and that we supposed, it contains a recombinant siderocalin (also known as Lipocalin 2, Neutrophils gelatinase-associated lipocalin, NGAL), it is expressed by macrophages and Neutrophiles in response to infectious and inflammatory stimuli.

Conclusion: These results suggest that Humoral and non specific immunity are influenced by iron deficiency and that the sequestration of iron-loaded siderophores by siderocalin represents a direct attack on an important iron acquisition mechanism.
